    Endovascular Therapy of Pseudoaneurysm and Arteriovenous Fistula in a Renal Graft

    A 31 year old male Caucasian received a renal cadaveric allograft. Reconstruction of an inferior polar artery was corrected pre-implantation. Delayed graft function occurred leading to dialysis support for one month. Graft biopsies(days 7, 15) showed acute tubular necrosis(ATN) and no rejection. Serial ultrasound (US), performed on average weekly, were compatible with ATN. On day 31, Doppler US and a CAT scan suggested for the first time a pseudoaneurysm adjacent to the implantation of the graft artery on the external iliac artery. For clinical and technical reasons, arteriography was only performed on day 67, when serum creatinine was 3.3 mg/dl. It showed a large pseudoaneurysm with an arteriovenous fistula to the right common iliac vein. Compression of the right external iliac artery was clear. In an attempt to close the arteriovenous fistula, the communication with the pseudoaneurysm was embolised with gelfoam and metallic coils with partial success. One week later, by right femoral approach a covered wallstent was placed immediately below the origin of the graft artery.Subsequent Doppler US and arteriography con-firmed closure of the communication with thepseudoaneurysm and of the arteriovenous fistula. The calibre of the right external iliac artery was then normal. By month 18, serum creatinine is stable at 2.1 mg/dl. We can only speculate on the origin of thepseudoaneurysm and of the AV fistula, whichwere not evident until one month post-transplantation. Backtable surgery was performed on thepolar not the main graft artery. Invasive angiography was irreplaceable in this unusual clinical situation

    Group B Streptococcus rectovaginal colonization screening on term pregnancies: culture or polymerase chain reaction?

    Objectives The aim of this study was to evaluate if screening Group B Streptococcus colonization by intrapartum polymerase chain reaction could improve intrapartum administration of antibiotic prophylaxis, compared with antepartum culture screening and analyze the sensitivity and specificity of polymerase chain reaction test. Methods 198 pregnant women with Group B Streptococcus colonization antepartum culture screening were included. When they arrived at hospital for delivery, two rectovaginal swabs were collected: for culture and polymerase chain reaction method. Results The rate of Group B Streptococcus colonization antepartum detected by culture was 16.7%; at delivery was 17.2% when detected by culture and 19.7% using polymerase chain reaction method. The rate of inconclusive polymerase chain reaction tests was 0.5%. Considering intrapartum culture screening as gold standard, sensitivity and specificity of polymerase chain reaction test for intrapartum Group B Streptococcus colonization was 97.1% and 95.7%, respectively. The global rate of discordance between antepartum and intrapartum Group B Streptococcus colonization was 6.6%. The rate of women not treated with intrapartum antibiotic prophylaxis in the setting of positive intrapartum culture was significantly lower using intrapartum polymerase chain reaction test (0.5%) than with antepartum culture method (3.5%, p = 0.035). Conclusion The use of intrapartum antibiotic prophylaxis can be more efficient when screening Group B Streptococcus colonization intrapartum by polymerase chain reaction test. Polymerase chain reaction method had a good performance in our study, with high sensitivity and specificity

    Echocardiographic evaluation including tissue Doppler imaging in New Zealand white rabbits sedated with ketamine and midazolam

    Limited data are available on the use of more recent echocardiographic parameters in the rabbit. Echocardiographic examination, including conventional echocardiography and tissue Doppler imaging (TDI), was performed on 26 male New Zealand white rabbits under ketamine-midazolam sedation. Particular emphasis was placed on the more recent systolic and diastolic parameters, such as myocardial performance index (Tei index) and mitral annular motion (from septal and lateral sides of the left ventricle) obtained using pulsed TDI. Parameters that assessed systolic and diastolic function (fractional shortening, Tei index, and maximal mitral E- and A-wave velocities) were comparable to those reported in the literature for rabbits in the awake state. The less cardiodepressive anaesthetic protocol could offer a good alternative in performing echocardiographic evaluation whenever such caution is necessary. TDI is feasible in healthy rabbits and potentially suitable for the investigation of left ventricle systolic and diastolic function

    Comparative analysis of the impact of new inspection regime on port state control inspection

    As an administrative measure to ensure maritime safety, Port State Control inspections are implemented and regarded as an important line of defence in coping with potential maritime accidents. To reinforce its role, a New Inspection Regime (NIR) was developed and put into practice by Paris MoU in 2011. It is widely recognized that the implementation of NIR has transformed and modernized the PSC inspection system in the Paris MoU region, stimulated the vessel qualit, and further improved vessel quality. In this paper, the influence of the implementation of NIR on the PSC inspection system and vessel quality is revealed for the first time. Based on inspection data and records collected from the Paris MoU online database, a comparative analysis between the ‘Pre-NIR’ (time before the implementation of NIR) and ‘Post-NIR’ (time after the implementation of NIR) periods is conducted from two perspectives. A macroscopic approach is first adopted to characterise the overall changes in inspection results and inspected vessels' quality through a statistical analysis of extrinsic Key Performance Indicators (KPIs) such as detention rate and deficiency rate. Then, a microscopic analysis is conducted to assess the influence of the NIR on intrinsic attributes of the vessels and of the inspection results based on Bayesian Network models derived from Pre-NIR and Post-NIR periods. The findings of this research systematically reveal the aspects from which the NIR improves the PSC inspection system, vessel quality and maritime safety. It will generate significant impact on and contribution to the promotion and stimulation of the adoption of NIR in more ports and regions to improve safety at sea in the whole world

    PZT-NZF/CF ferrite flexible thick films: Structural, dielectric, ferroelectric, and magnetic characterization

    The preparation and properties of thick flexible three-phase composite films based on lead zirconium titanate (PZT) and various ferrites (nickel zinc ferrite (NZF) and cobalt ferrite (CF)) were reported in this study. Properties of three-phase composite films were compared with pure polyvinylidene fluoride (PVDF) and PZT-PVDF films. X-ray diffraction data indicated the formation of well crystallized structure of PZT and NZF/CF phases, without the presence of undesirable phases. Scanning electron micrographs showed that the ceramic particles were dispersed homogeneously in the PVDF matrix and atomic force microscopy confirmed that the size of the particles is around 30 nm. Non-saturated hysteresis loops were evident in all samples due to the presence of highly conductive ferrite phases. Under magnetic field of 10 kOe, composite films exhibited a typical ferromagnetic response. Dielectric properties were investigated in the temperature range from -128 to 250 degrees C and frequency range of 400 Hz-1 MHz. The results showed that the value of dielectric constant of the PVDF/PZT/ferrite composites increased about 25% above the one obtained for pure PVDF

    Case Report: Pheochromocytoma and Synchronous Neuroblastoma in a Family With Hereditary Pheochromocytoma Associated With a MAX Deleterious Variant

    Introduction: Pheochromocytomas are rare catecholamine-producing neuroendocrine tumours arising from chromaffin cells of the adrenal medulla or extra-adrenal sympathetic paraganglia. Recent studies have indicated that up to 40% of pheochromocytomas could be attributable to an inherited germline variant in an increasing list of susceptibility genes. Germline variants of the MYC-associated factor (MAX) gene have been associated with familial pheochromocytomas and paragangliomas with an autosomal dominant pattern of inheritance, a median age at onset of 33 years and an overall frequency estimated at 1.9%. We describe a deleterious MAX variant associated with hereditary pheochromocytoma in a family with four affected individuals. Case presentation: The first patient presented with bilateral pheochromocytoma in 1995; genetic testing was proposed to his oldest son, when he was diagnosed with a bilateral pheochromocytoma with a synchronous neuroblastoma. Upon the identification of the MAX variant c.97C>T, p.(Arg33Ter), in the latter individual, his two siblings and their father were tested and the same variant was identified in all of them. Both siblings were subsequently diagnosed with pheochromocytoma (one of them bilateral) and choose to remain on active surveillance before they were submitted to adrenalectomy. All the tumours secreted predominantly norepinephrine, accordingly to the typical biochemical phenotype ascribed to variants in the MAX gene. Conclusion: This case series is, to our knowledge, the one with the largest number of individuals with hereditary pheochromocytoma with a deleterious MAX variant in the same family. It is also the first case with a synchronous pheochromocytoma and neuroblastoma in carriers of a MAX deleterious variant. This report draws attention to some ill-defined features of pheochromocytoma and other malignancies associated with a MAX variant and highlights the importance of understanding the genotype-phenotype correlation in hereditary pheochromocytoma and the impact of oriented genetic testing to detect, survey and treat patients and kindreds at risk
